Care Synergy and CAPABLE National Center Named Finalists for Elevance Health Foundation Patient Safety Prize

Care Synergy and the CAPABLE National Center are proud to announce that we have been selected as finalists for the Elevance Health Foundation Patient Safety Prize.

The recognition highlights innovative, proven solutions with the potential to improve patient safety for communities across the United States. Care Synergy and the CAPABLE National Center were selected as finalists in the prize's Promoting Fall-Free Futures category.

A national focus on patient safety
The Elevance Health Foundation launched the Patient Safety Prize in December 2025, committing up to $5 million to support solutions addressing three important areas of patient safety:

Innovating to Eliminate Medication Errors
Promoting Fall-Free Futures

Falls are a significant concern for older adults and can have a profound impact on health, independence and quality of life. Preventing falls requires more than addressing a single medical issue. It often means looking at the individual, their home environment and the everyday challenges that can make it difficult to move about safely and independently.

That is where CAPABLE can make a difference.

Helping older adults live safely and independently at home
CAPABLE, which stands for Community Aging in Place – Advancing Better Living for Elders, is an evidence-based approach designed to help older adults remain safely and independently in their homes.

The program brings together the expertise of occupational therapists, nurses and trained home-care workers to identify an individual's goals and challenges and develop practical solutions. These can include addressing safety concerns in the home, supporting mobility and daily activities, and making modifications that help reduce barriers to independence.

By focusing on what matters most to each individual, CAPABLE takes a person-centered approach to improving safety and quality of life.

An opportunity to advance the work
As one of five finalists for the Patient Safety Prize, Care Synergy and the CAPABLE National Center will have the opportunity to incorporate feedback from the Peer Review and Evaluation Panel to further strengthen and refine our project.

Finalists will also create a pitch video and provide a project update for consideration by the Selection Committee. Following a virtual pitch event, the Selection Committee will recommend up to three winners, each eligible to receive up to $1.5 million, as well as up to two Honorable Mentions, each eligible to receive up to $250,000.

The winners and Honorable Mentions are expected to be announced in fall 2026.

We are honored to be recognized alongside four other finalists: NYU Grossman School of Medicine, Parkland Center for Clinical Innovation, The Population Health Innovation Lab, a program of the Public Health Institute, and the University of Pittsburgh.

Looking toward a safer future
Being named a finalist is an exciting milestone for Care Synergy and the CAPABLE National Center. More importantly, it provides an opportunity to shine a national spotlight on the importance of helping older adults remain safe, independent and engaged in the places they call home.

We are grateful to the Elevance Health Foundation for this recognition and for its investment in innovative approaches to patient safety. We look forward to continuing this work and sharing more about the potential of CAPABLE to help create fall-free futures for older adults across the country.
